One of the most challenging steps in the surgery of the rectum is the section and closure of the rectal stump in order to perform an anastomosis (colorectal rejoining) that avoids a colostomy. Ideally, the intestinal verge is pukered around the axis of the anastomosing circular stapler. In the narrow pelvis, this becomes challenging in the rectum. There are two alternatives: 1. Rectal closure with a linear stapler. Is the most common option that, nevertheless, leaves extra suture line and poor vasculairzed areas that can increase the risk for an dreadful anastomotic failure. 2. Transanal surgery. Technically challenging and associated to severe complications. We propose a solution that combines a properly modified circular stapler with a surgical thread to create a pursing suture at the verge when cuting the rectum. It is technically easier than available alternatives, avoids low vacularized areas and suture excesses and provides a higher quality surgical specimen for pathological study. The proof of concept performed with very little tools worked well; now it's time for surgical stapler manufacturing companies to move it to clinical practice.
